ICU delirium trials are flawed based on your astute observation that hyper vs hypoactive deliriums are just not the same entity. There is a complex myriad of difference in neurotransmitters and interactions with neuros which require a corresponding pharmacological intervention which we lack the tools to correctly assess each tjme simply because - we cannot measure neurotransmitters and neuronal interactions readily. This problem has and will contiued to plague the world of neuropathy for the foreseeable future until technically catches up. Until then I don't really take any "positive" ICU delirium trials that seriously
