I think you have missed the point! The paste does nothing ....it is the 4 days of intravenous antibiotics that is the effective part of SDD! And remember, apparently the George Institute has some sort of patent over the paste, so I imagine there is an enormous conflict of interest in the publication!

You comment prompted me to recall reading a meta-analysis way back in which the purported survival benefit was limited to patients who received IV antibiotics (ie no benefit in truly selective decontamination alone). This would of course make the whole process much easier and available for anyone to implement immediately ‘off the shelf,’ even front line maverick intensivists who don’t mind toeing up against their PharmD and infection control people. One could just provide 4 days of ceftriaxone IV and be done with the “protocol”. I will look into the hypothesized/ alleged conflicts—great insights, thanks.
